During a patient or candidate’s first visit to our clinic they will be asked to complete a thorough medical history for the services being sought. To expedite your visit some of these forms are available on this website under patient forms. You may download these and complete then bring with you to your appointment. All visits to our clinic must have a completed employer treatment request -authorization form which can be found in the forms section of this website.
For employers new to our services, we have provided a company profile form which can be completed and emailed to us. There is also an employer treatment request -authorization form which must accompany all employees/candidates to the clinic.
What To Expect
Be prepared when you come to your appointment with proper documents and picture identification along with any supporting documents required for your visit. Timely completion of registration documents and authorization requirements results in little wait time for your visit.
